One of North Carolina’s Top ACOs Aligns Referrals Management Across Community Provider Network
GREENSBORO, N.C. – July 7, 2021 – Today Proficient Health™ announced that WakeMed Key Community Care (WKCC), an Accountable Care Organization (ACO) in Raleigh, NC, has signed a multi-year contract with Proficient Health to align referrals across its primary care physician practices and specialty providers.
The implementation of PH Exchange™ means WKCC practices can create, track, and manage referrals in a centralized system — one that works with provider systems, regardless of EHR software. Practices and providers will have full visibility throughout the referral process and can easily communicate to address follow-up notes or provide other clinical information.
WKCC now has access to all products within the Proficient Health platform to optimize processes for urgent, emergent, and routine referrals for 430+ primary care providers and specialists within the ACO.
Debi Hueter, executive director at WKCC said it was important from a technology standpoint to find a solution that would seamlessly connect their complex network working with multiple EHRs and meet providers in their current state.
